Bring your Beacon Card to sign up.
If you don’t have a Beacon Card
*We will need Picture ID for all adults in the household.
*Social Security card for all in household
*Proof of Bourbon Co. residency
*Proof of Income
* If you receive any service from DCF must sign a release for verification
